He trained in the United States as a doctor of physical therapy, then pursued advanced certification in dry needling, Functional Patterns biomechanics, the Wim Hof Method, and Pancafit global postural work — a body of training that goes well beyond what a standard PT license requires or a standard PT clinic offers.

He treats with his hands first. He explains what he finds while he is finding it. The diagnosis and the treatment happen in the same room, in the same hour, with the same person — which is rarer than it should be.

High-performance athletes have bodies that are simultaneously overtrained and under-recovered, and they cannot afford to lose weeks to a treatment approach that guesses. The Physica Medica Approach

The body is continuous. Fascia does not stop at the border of the muscle it wraps. A restriction in one region loads the next. Pain at the surface often originates somewhere else entirely — which is why treating only the site of pain produces only temporary relief.

A tight jaw can hold the tongue. The tongue can hold the diaphragm. A compressed diaphragm changes how the ribcage moves, how the spine loads, how the hips rotate. Follow that chain far enough and you find the source. That is the work.

What makes Dr. Birikov different from other physical therapists in Baltimore?

Every session is a full hour. One clinician. No assistants, no aides, no exercises handed off to someone else while Dr. Birikov moves to the next room. The hands-on work happens for the full duration of the visit, which is not the standard model at most PT clinics and is the main reason results here differ from results elsewhere.

What credentials and certifications does Dr. Birikov hold?

Physica Medica is out-of-network by design. Insurance reimbursement rates require volume — more patients, shorter sessions, less time per problem. That model works for straightforward cases.
