Concierge, hands-on physical therapy in Baltimore for people who have already tried PT, massage, and chiropractic and are still in pain.
Dr. Maks Birikov, DPT
Baltimore's first master dry needler
Full-hour, one-on-one sessions in Fells Point.
You have been to a physical therapist. Probably more than one. You were handed a sheet of exercises, given fifteen minutes with an aide, and booked for a follow-up. Maybe you tried massage. Maybe chiropractic. Something released for a day or two, and then it came back.
That is not a failure of effort. It is a failure of address. Nobody found where the problem actually lives.
A tight jaw can hold the tongue, the tongue can hold the diaphragm, and a diaphragm that cannot move will keep a neck, a rib cage or a low back locked no matter how many times you stretch it. Chains like that are why the place that hurts is so rarely the place to treat.
There is still a lot you don't know about your problem. That is precisely why you haven't gotten better yet, and why there is still a straightforward way forward.
Stated as facts, not claims. Each is verifiable, and each one changes what can be done in a treatment room.
Master-level certification. Dry needling sits within the licensed scope of physical therapy practice in Maryland.
Biomechanics retrained around gait, breathing and rotation. Not isolated muscle drills.
Breathwork and cold exposure taught as a physiological protocol, run in small groups.
Global postural decompression, developed in Italy and rarely taught outside Europe.
Hundreds of professional athletes have been treated in this room, including players from the Ravens, the Orioles and the Baltimore Blast, alongside patients who arrived after years of physical therapy that never held.
Every session is a full hour, one-on-one, with Dr. Birikov. No aides, no rotating staff, no exercise handout in place of treatment. The modality is chosen for your tissue, not for a billing code.

Some problems are missed because they are rare. Most are missed because the origin sits somewhere nobody thought to examine.

Dr. Maks Birikov, DPT, built Physica Medica around one constraint: the hour belongs to the patient.
He trained in the United States as a doctor of physical therapy, then went looking for what American programs do not teach. Pancafit in Italy, European fascial and neural release, master-level dry needling, Functional Patterns biomechanics, the Wim Hof Method. He treats with his hands first, and he explains what he finds while he is finding it.
Athletes arrive mid-season, when there is no time for guessing. Others arrive after two years of physical therapy that never produced results. Most patients feel meaningful change within four to six sessions; where a case is genuinely complex (scoliosis, decade-old fascial restriction, post-surgical adhesion) he will give you the arc after he has his hands on you, not before.

Documentation, not adjectives. Posture, curve and gait are photographed at intake and re-shot as the work progresses, under the same conditions every time.
Video comparisons for gait and breathing cases are shown in consultation.

Fascial release along the concavity, then breathing retraining into the collapsed segment. Rotation reduced and the rib prominence visibly settled. Same room, same markings, same light.

Anterior chain and thoracic fascia released by hand, then loaded so the position holds without cueing. Ear, shoulder and hip back on one line.
You see Dr. Birikov, for the full hour, every visit. The work is manual: fascia, joint, nerve, needle, rather than a set of exercises supervised by an aide. And the credential stack behind it exists nowhere else in the city: first master dry needler in Baltimore, first Wim Hof instructor, the only Functional Patterns practitioner, the only Pancafit class in the country.
Physica Medica is out-of-network by design. Reimbursement rules dictate session length, staffing and modality, which is exactly why insurance-based clinics cannot deliver an hour of hands-on care with the doctor. Sessions are paid out of pocket; HSA and FSA funds are accepted, and a superbill is provided if you wish to file for out-of-network reimbursement. Rates are quoted plainly before you book.
You have tried physical therapy, massage or chiropractic and you are still in pain. You want hands-on treatment, not an exercise sheet. You would rather have one hour with an expert than three appointments with three people. If that describes you, the consultation will tell you quickly whether this work applies to your case, and Dr. Birikov will say so if it does not.

Send the story you are tired of telling: what hurts, what you have already tried, and what nobody has been able to explain. Dr. Birikov reads every inquiry personally and responds directly. There is no front desk and no scheduling service between you and the answer.
The studio sits on South Bond Street in Fells Point, a few minutes from Harbor East and Canton. Street parking is available on Bond and Thames; arrive five minutes early for your first session.
