How is Physica Medica different from a regular physical therapy clinic?
The hour is not shared. You see Dr. Birikov, a doctor of physical therapy, for sixty uninterrupted minutes, every visit, and the work is manual: fascia, joint, nerve, needle. There are no aides, no staggered patients and no printed exercise sheet in place of treatment. Behind that hour is a credential stack that exists nowhere else in the city.
Who is Dr. Birikov?

Do I need a referral, or do I have to be an existing patient?
Neither. Maryland is a direct-access state, so you can book without a physician referral, and new patients are welcome. If your case needs medical imaging or a specialist opinion first, Dr. Birikov will tell you that rather than treat around it.

Will the treatment hurt?
Deep manual work can be intense while it is happening. The sensation patients describe as "good pain," the kind that makes you exhale. It is not endured in silence: Dr. Birikov talks through pressure as he works and adjusts on request. Sharp, guarding pain is information, not a target, and the work changes when it appears.
Will I be sore afterwards, and can I still work?
Expect a day or two of soreness after deeper sessions, closer to the ache of hard exercise than to injury. Most patients go straight back to work. Sessions can be scheduled so the heaviest work is not the day before something that matters.
Will cupping leave marks I have to explain?
Sometimes, yes. Myofascial cupping can leave circular discolouration where the fascia was decompressed; it is not a bruise, and it typically fades within three to seven days. Is dry needling safe, and can a physical therapist legally do it in Maryland?

Do you accept insurance, and how does pricing work?
Physica Medica is out-of-network by design. Reimbursement rules dictate session length and staffing, the exact two things that make this work possible, so insurance-based clinics structurally cannot deliver a full hour of hands-on care with the doctor. Sessions are paid out of pocket; HSA and FSA funds are accepted, and a superbill is provided if you wish to file for out-of-network reimbursement. Rates are quoted plainly before you book. Is a boutique clinic worth it compared to a standard PT clinic?
Compare what an hour buys. At a standard clinic, roughly ten minutes of it is skilled hands-on time and the rest is supervised exercise. Here, sixty minutes are skilled hands-on time with a specialist. Patients who arrive after two failed courses of conventional care are usually not paying more overall. They are paying once instead of three times.

What is IASTM?
Instrument-assisted soft tissue mobilization. Steel instruments are drawn across the tissue; their edges pick up adhesion and thickened fascia that fingers glide over, and the controlled friction breaks that scar-like tissue down so the layers move independently again. About IASTM →
What is myofascial cupping?
The opposite of massage. Instead of pressing tissue down, negative pressure lifts fascia away from the layers beneath it, decompressing the space and restoring both glide and blood flow. It is particularly effective where a whole region has become stuck as a sheet. About cupping →
What is the difference between dry needling and acupuncture?
The needle is similar; the reasoning is not. Acupuncture works from meridian theory. Dry needling is Western musculoskeletal practice: a filament needle is placed into a specific trigger point in a specific muscle to produce a twitch response and release it at a depth pressure cannot reach. What is Functional Patterns, and what is Pancafit?
Functional Patterns retrains gait, rotation and breathing as one movement system rather than a list of muscles to strengthen. Dr. Birikov is the only practitioner of it in Baltimore. What does "fascial rehabilitation" actually mean?
Fascia is the connective sheet that wraps and links every muscle. When part of it thickens or adheres, the restriction transmits along the sheet, which is why a hip can be held by an old abdominal scar. Prior failure is a clue, not a verdict.
Three failed courses of treatment usually mean the origin was never located, not that the problem is untreatable. Bring that history with you. It is diagnostic, and it shortens the search.

How many sessions will I need before I see results?
Most patients feel meaningful change within four to six sessions. Recent, localized problems often resolve in two or three. Chronic postural patterns, scoliosis and long-standing fascial restriction typically need a longer course. A decade of adaptation does not unwind in a fortnight. You get a specific range after the first assessment, not a package sold before it.
What determines how long treatment takes?
Three things: how long the problem has been there, how many compensations have been built on top of it, and what happens between sessions. A two-month shoulder with one compensation is a short course. A twenty-year back with a rotated pelvis, a locked rib cage and an old surgical scar is a project. And it is still solvable.
I have tried everything and nothing has worked. Is this different?
That describes most of the practice. The reason to expect something different is not enthusiasm. It is method: a full hour of hands-on assessment across the whole chain rather than fifteen minutes at the site of the pain. If after the first session Dr. Birikov does not believe he can help, he will say so directly.
Could treatment make my condition worse?
Manual therapy is graded to the tissue in front of it, and the assessment comes first precisely so the work matches the case. Where a condition needs imaging, medical management or surgical review before hands-on treatment, Dr. Birikov refers rather than proceeds.
