Request a ConsultationOne doctor. One patient at a time. No aides, no handouts, no shared hours.

Harbor Point was built for people who expect things to work well. The clinic two minutes away operates on the same standard. Physica Medica is a concierge manual therapy practice. Not a volume clinic. Not a gym with treatment tables.
If you work near the Exelon building or live along the waterfront and you've already tried physical therapy, massage, or chiropractic without lasting results, the issue usually isn't that treatment failed. It's that the treatment addressed the symptom and skipped the chain that produced it.
Dr. Birikov works differently. Every session is a full hour of direct, hands-on treatment. He traces the restriction to its origin, not just where it hurts.
Every service below is performed by Dr. Birikov personally. No aides, no assistants, no shared appointments.

Learn more →A fine filament needle is inserted into a trigger point to release the contraction and restore normal tissue tone. Dr. Birikov holds master-level certification, the first in Baltimore to do so. Dry needling is within the licensed scope of physical therapy practice in Maryland.
Learn more →Suction cups lift the fascia away from the layers beneath, creating space where tissue has been compressed or stuck. Cupping can leave temporary marks on the skin that typically fade within a few days.
Learn more →A methodology that retrains gait, rotation, and breathing as one connected system rather than isolated muscles. Dr. Birikov is the first and only certified Functional Patterns practitioner in Baltimore.
Learn more →A global postural decompression method developed in Italy. Physica Medica runs the only Pancafit class in the United States.
Learn more →European fascial release techniques applied to restrictions, adhesions, and scar tissue from surgery that limit movement and generate referred pain.
Learn more →Jaw tension pulls on the tongue. Tongue tension restricts the diaphragm. The voice suffers last, but the problem started much earlier. This work treats the full chain.
Learn more →Curve-specific fascial work combined with breathing retraining to address the structural and neuromuscular components of scoliosis.
Learn more →Clenching, clicking, facial pain, and jaw restriction treated with both external manual therapy and intraoral buccal work.
Learn more →Harbor Point draws a specific kind of resident: younger professionals, active people, desk workers who also train. The conditions that follow them into the clinic reflect that.
The most common profile at Physica Medica is someone who has already been through a course of PT, seen a chiropractor, tried massage, and is still in pain. That's not a coincidence. It's a referral pattern. When standard approaches don't resolve the problem, the problem usually has a different origin than anyone looked for.
Forward head posture, chronic neck tension, and low back pain that builds through a workday are structural problems, not effort problems. Sitting more carefully won't fix them. Hands-on work that addresses the fascial restrictions driving the posture will.
Dr. Birikov has worked with players from the Ravens, the Orioles, and the Baltimore Blast. That same level of assessment and manual therapy is available to Harbor Point residents, recreational athletes included.
Vocal tension rarely starts at the voice. It starts at the jaw, moves through the tongue, and reaches the diaphragm. Dr. Birikov treats the full chain, not just the symptom the performer notices.

Dr. Birikov holds a doctorate in physical therapy and is licensed in Maryland. His advanced training extends to European fascial rehabilitation methods not commonly taught in the United States.
Master-level certification in dry needling, the first in Baltimore to hold it. Dry needling is within the licensed scope of physical therapy practice in Maryland.
Functional Patterns retrains movement around gait, breathing, and rotation rather than addressing muscles in isolation. No other practitioner in Baltimore is certified in this methodology.
Breathwork and cold exposure taught as a physiological protocol, not a wellness trend. Dr. Birikov was the first certified Wim Hof instructor in Baltimore.
Pancafit is a global postural decompression system developed in Italy. Physica Medica is the only place in the country where it is currently taught.
From the Exelon campus or Thames Street Wharf, the clinic is roughly a five-minute walk south along the waterfront into Fells Point. By car, it's two minutes. Street parking is available on Bond Street and Thames Street. The address is 800 S Bond Street, Baltimore, MD 21231.
No referral is required to book. New patients are welcome directly. If you've been told you need a referral before seeing a physical therapist, that applies to insurance-based clinics. Not here.
Physica Medica does not bill insurance. Treatment is paid out of pocket. That structure is what makes one-on-one, full-hour, doctor-only care possible, and it's why the model exists.
Most patients with straightforward musculoskeletal conditions see meaningful change within 4 to 6 sessions. Complex or chronic cases take longer, and Dr. Birikov will tell you that directly after an initial assessment rather than giving a number that doesn't apply to your situation.
Tell Dr. Birikov what hurts, what you've already tried, and how long it's been going on. That context matters before the first session.
Dr. Birikov reads every inquiry personally and responds directly. Call 443-228-8029 or send a message through the contact form.
